JavaScript - Can't Figure Out Javascript Error
Need help with the Script below. I got my radio buttons working, however...the rest of my form/webpage now won't appear. Advice? Please.

My question! How do I emulate this on my website? I have the Yext service. I have been on the phone with the company for over a week now and I cant get any technical support on the integration. I am getting an error with a script and can't seem to figure out the problem. Firebug Error message: Error: document.forms[myform] is undefined Source File: https://www.domain.com/js/remember.js Line: 24 I have attached the HTML page and the remote javascript file.
