PHP - Php Form Mail Not Sending Mail

Everything in my mail form is fine and the mails do get send. But they don't always arrive (checked spam folder). Hotmail or example arrives but a normal email adres from a provider does not. Could this have something to do with SMTP ? could I add this to my code ? When I open this in the browser, it says "Query is empty". Anyone want to help a newb out and show me what I'm doing wrong? I have removed the emails, host, port, username and password along with in other information I did not think I should send over.
