PHP - Not Understanding A Line Of Code From A Simple Cms Program

I'm trying to work through a very simple CMS that I found on the web (just for learning purposes). I need some help understanding this line: <table> <form method="POST" action="account.php?mode=save<?=( isset($_REQUEST['id']) ? "&id={$_REQUEST['id']}" : null )?> <? I understand the Method = "POST", but not the "action = " line. Thanks for any help. Similar TutorialsI recently converted my business to an ip-pbx that has lots of capabilites. Among them is a feature I would like to use called http_notify. It sends a post to a url with parameters such as caller id number, caller id name etc.. I need to write a simple program to accept these parameters and write them to a csv file along with a date/time stamp. This seems like a super easy, short piece of code to come up with and as a noob I was hoping to write this myself. I have found a few examples where the post is for a form but I can't find any examples where the data is coming in as part of the url. This is the description of the feature on the phone system site: HTTP Notify Sends notifications to a remote web server through a URL. This allows allows an event in a Jive Core PBX to trigger an action on a remote system. Simply enter a URL in the node, and that URL will be hit with an HTTP POST as calls 'pass through'. Multiple variables are available and all variables are also included in the post body. URL Defines the URL to be used (limit of 255 characters). Next Node Connects nodes together to create a call flow. Drag and drop on the next node in the dial plan. I'm just trying to incorporate some of what I've learned into practice and I can't believe I'm having problems with this but I really want to figure it out. I'm trying to create a simple program where a user enters the name of a song, and gives it a rating (out of 5), and it displays the output first by song rating then (if the ratings are the same) displays it by the name of the song alphabetically. The idea in practice would work like this...user enters: Quote Song Name: My Song Rating: 3 Song Name: His Song Rating: 5 Song Name: Her Song Rating: 5 Your list so far is: Her Song - 5 stars His Song - 5 stars My Song - 3 stars I just keep undoing things and clearly I'm not getting it. If I could figure out how to use these session arrays to hold the entries from the post, then I'd be able to sort them properly, I'm just lost right now because everything I've read online and tried wasn't fixing the problem. For example with an IDE to run a program is there a way to put some code in the path and let it tell me where it came from like a referral? $ref = $_SERVER['HTTP_REFERER']; echo("$ref<br>"; Result: https://www.mydomain.com/wp-admin/admin.php?page=lambo As you can clearly see the page that we have entered the code in is referred by the URL here. It tells me that it was last at a word press page /wp-admin/admin-php and I can go to that page and it will be the last page it was at.. So bottom line then if i add this code to the admin page and click the same button i had before.. it will tell me the file that called out the admin page. Baslcaly I am looking for ideas on how to track the page i am now all the way back to the page with the button on it and all intermediate pages that it passes thru to get to the final destination. It seems that to succesfully troubleshoot a fault is to follow the path of the entire job and see if any breaks... Any thoughts or suggestions on how to best do this would greatly be appreciated.

I've cloned a mysqli_connect() statement in a 'require' file placed in a non-public access location on the server. It's the same format (and location) currently working fine with another application, EXCEPT i changed the database name to what I need to use for this application, AND added the $con= part on the front end to substitute for the commented out $con= line in the example code I'm modifying to insert data into a new MySQL DB Table.
The dB Table is fine and I have Inserted data via myPHPAdmin with no problems. Only two form variables are in the simple PHP script thus far just to test if it will work before proceeding.
I've used pseudonames for the actual /home/.... location and filename, and am now Baffled about the Line 2 Error Message I'm getting (below the code).
