One of the chores it must do is collect information from the database and put it in a list/menu select box on a page so the user can choose which item to pursue.
In the following (incomplete) snippit, I am doing something incorrectly because the sql query does get the proper information (I can put it in a table on the page just fine). But I'm having trouble getting the information into the select options on a list/menu. It appears to be putting them all, one after the other in the first option spot. The last one seems to be the only one of 6 or 7 that shows up.
It's been 10 or 12 years since I've messed with php, so I think I'm way behind... any help would be appreciated.

Heres where I get stuck.... as I am constantly adding to the list I have no fixed amount of items so the form for the merchants needs to be built dynamically, ie each time I add a new material, I want the merchants form to get a new input. I am not very good at describing my problems but I am hoping that someone can help me out from the information given. Thats the first question. i like to get issues sorted in small bites. the second is the next big bite, I want to send the form above to several merchants, so I intend to have a user database with a user for each merchant, and link the tables so that each merchant can provide a price for each material, ideally the form would highlight the materials that have not had a price yet. I'm looking for advice on the best method of doing this basically but if I can get the first question sorted that would be great.. I have the test site local but can upload it it to the server if it will help you see what im trying to do. Any help that anyone can give will be greatly appreciated. If the user clicks 'Book Now' on Event #1 I need it to take them to the HTML/PHP booking form with the information already entered that is relevant to Event #1 (Or whatever event they choose), such as the event name, time, date etc I know how to create a contact form etc, its just the carrying of the information over to the form I dont know how to do. I am trying to retrieve info. from mySql database and update 3 textfields.
