PHP - Foreach W/ If And Else On Checkboxes - Cannot Get This Working Without Errors

But I read a thread earlier and I am working on a dynamic way to address checkboxes with some intuitive messages while processing, and I cannot get this to work right w/out errors.. I'm trying to figure out how to process the form once submitted. I've got a foreach loop set up, but I need to know the id of the checkbox so that I can make the change to my database accordingly. At the moment, my form is built with the checkboxes given the name of "checkbox['$id']", where id is pulled from the database. and yet I use everything in the for correctly.
Please help me out. I have spent hours looking for an answer.
It also posts the checkboxes I select at the bottom. I just want to be sent to the email, not necessarily echoed so I can view it.

As result/row is retuned to the user i would like to be able to detect whether or not they have checked a checkbox at the end of each rown in which case multiple rows can then be deleted from the database upon the user clicking a 'delete multiple records' button. I have no problem in being able to display the checkboxes on the webpage but i am a little unsure as to how to refernce them and detect which ones the user has checked. Given my database stores hostnames/IP addresses would it be best to name each checkbox to reflect the hostname or name the value it returns when checked to reflect the hostname. Given the above and that I also store the results of a users search in a $_SESSION['hosts'][][] array how can I then tie the two together? Thanks for reading. If I miss a semicolon, I get: "HTTP Error 500.0 - Internal Server Error" absolutely useless. In prior versions, I could see the line and get to the PHP error. How do I display PHP errors?
