PHP - Enter Birthdate Into Database From Form
How can I build a form with a drop down menu with the year, day, month? And then send that information to a database.
If there is another/better option besides a drop down menu? If so, please tell me. The rest of the form is working fine, and all the inforamtion is going to the database - last name, first name, address, phone number, zip code, race,,,,,,,,,. I work for a not-for-profit medical office - we provide medical services to the local people who can not afford to go to a regular doctor. We need a simple way to keep track of patient information. One of the things we need to enter is the birthdate. Searching the forums provided a lot of results, but the words date and form are used so much, I can not find anything that helped me. I also looked through php.net, but can not find anything there. Similar TutorialsThe company I work for is a not-for-profit organization, we provide medical services for people who can not afford to go to the regular doctor. We need a way to keep track of our patients and their visits and a way to search the data. I found an html form, with name, address, phone number, zip code,,,,,, and made a few changes to fit our needs. But I do not know how to get that information into the database. I also need a way to make sure the information is not a duplicate. Since we are a not-for-profit organization, funds are kinda tight. That is why I want to go with php instead of buying medical software. Most of the medical software on the market cost more then we can afford. I'am a computer tech here at the office, but my field is hardware, networking, and malware removal. PHP programming is new to me. So where do I start? Not sure how to even search for what I'm looking for, which is to say I'm really in need of guidance. I have a pre-populated database that I set up with about 260 names in it. It also has information like, height (basketball players) and high school. It's a list of kids I mean to invite to play in a league. From there, they will actually come to the league's site and register. I'd like for my form to check to see if they are already in the database, and if they are just add personal information, like email and phone number. If they are not, I'd like for the form to enter all of the information (name, school, etc). less than 6 characters. I think it's the way my code is ordered. I've tried switching the commands around, no luck. Help please. Code: [Select] <?php //begin register script $submit = $_POST['submit']; //form data $username= strip_tags ($_POST['username']); $email= strip_tags($_POST['email']); $pwd= strip_tags($_POST['pwd']); $confirmpwd= strip_tags($_POST['confirmpwd']); $date = date("Y-m-d"); if ($submit) { //check for required form data if($username&&$pwd&&$confirmpwd&&$email) { //check length of username if (strlen($username)>25||strlen($username)<6) { echo "<p class='warning'>username must be bewteen 6 and 25 characters</p>"; } else { //check password length if (strlen($pwd)>25||strlen($pwd)<6) { echo "<p class='warning'>password must be between 6 and 25 characters</p>"; } else { //register the user echo "<p class='success'>Thanks for signing up!</p>"; } } //check if passwords match if ($pwd==$confirmpwd) { } else { echo "<p class='warning'>your passwords do not match</p>"; } //encrypt password $pwd = md5($pwd); $confirmpwd = md5($confirmpwd); //open database $connect = mysql_connect("xxxxxxxx", "xxxxxxxx", "xxxxxxxx"); mysql_select_db("digital"); //select database //register the user $queryreg = mysql_query(" INSERT INTO users VALUES ('','$username', '$email', '$pwd') "); die("<p class='success'>Thank you for signing up you have been registered"); } else { echo "<p class='warning'>please fill in all fields</p>"; } } ?> Before I go digging around for code or conversion to unix time stamp, I'm curious how many of you have a birthday entry on a form which is the least hassle for the person filling out the form. For example a text box and they can enter 12/1/1992 or 12/1/92 or 12-1-1992 or three text boxes for month, day and year and if so, require an 01 or a 08 or 4 characters for year. The second method with three boxes would easily convert to a date stamp entry on the other hand if there is a php conversion that would convert any format (12/1/1992, 12/01/92, etc) to a unix time stamp that would make it easier for the person filling out the form. Thanks in advance for any ideas or help. Mike Hello PHP world! I have this roll-down-select-menu working, reflecting a column in my database. I am able to retrieve the selected value after submit, that takes me to another code page. But I would like to use this value on the form page, that contains the roll-down-menu and the submit button. Is there a way to enter the chosen value into this variable $uan, without having to submit the page first? I tried to retrieve the value on this page with the POST lines at the button, but without luck. I think the value is only entered upon submit? Thanks in advance for your clever advice. Best regards Morris <?php $query = "SELECT * FROM Uniq_artist_name" ; $result = mysql_query($query); echo'<select name="uan">'; while($row = mysql_fetch_assoc( $result )) { echo '<option value="'.$row['uniqartistname'].'">'. $row['uniqartistname'] .'</option>'; } echo '</select>'; $uan = $_POST["uan"]; echo "$uan"; ?> Hello everyone, I have a page with a short form with just a password field and a submit button. I also have php code setting variables depending on whether or not the password is correct. When the password is entered and the submit button is CLICKED, everything works as it should. However, when the password is entered and the ENTER button is pushed to submit the form, the form is reset and nothing else happens. Also, this only happens in I.E., it works fine with Firefox. I have plenty of other forms that I use in the same manner but they all work whether the submit button is clicked or the enter button is pushed to submit, regardless of what browser is used. The idea is to have this window close (it's opened as a pop-up) and the main window load the next page. Again, it works perfectly as long as the submit button is clicked but not if the enter button is pushed. Not sure if it has something to do with my php code or what. Any ideas on how to fix this? Here's my code: Code: [Select] <?php if (isset($_POST['submit'])) { $pword = md5($_POST['pword']); $q = mysql_query("SELECT * FROM `accounts` WHERE acct_number = '$acct' AND pword = '$pword'") or die (mysql_error()); $r = mysql_num_rows($q); // Checks to see if anything is in the db. if ($r == 1) { $student = "true"; } else { $student = "false"; } } else { $student = "unknown"; } ?> <!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd"> <html xmlns="http://www.w3.org/1999/xhtml"> <head> <meta http-equiv="Content-Type" content="text/html; charset=iso-8859-1" /> <title>Student Verification...</title> </head> <!-- <body> tag is in PHP --> <?php if ($student == "true") { echo "<body onunload=\"opener.location=('signup.php?acct1=$acct')\">"; } else { echo "<body>"; } if ($student == "true") { echo "<div align='center'>"; echo "<p><font size='+2'>Passcode Correct!</font></p>"; echo "</div>"; echo "<div align='justify'>"; echo "<p>You will now be directed to the signup page.</p>"; echo "</div>"; echo "<div align='center'>"; echo "<p>Thank you.</p>"; echo "<input type='button' value='Continue to Signup...' onClick='window.close()'>"; echo "</div>"; echo "<meta http-equiv=\"REFRESH\" content=\"20;url=close.php\">"; } elseif ($student == "unknown") { echo "<div align='center'>"; echo "<table width='325' height='175' border='0' cellspacing='0' cellpadding='0'>"; echo "<tr>"; echo "<td width='325' align='center' valign='middle'>"; echo "<p><font size='+1'><strong>Welcome!</strong></font></p>"; echo "<p>Please enter the school passcode.</p>"; echo "<form name='form' method='POST' action='schoolpw.php'>"; echo "<p><input type='password' name='pword' size='30' maxlength='20' /></p>"; echo "<input type='submit' name='submit' id='submit' value='Submit' /> "; echo "<input type='button' name='cancel' value='Cancel' onClick='window.close()' />"; echo "</form>"; echo "</p>"; echo "</td>"; echo "</tr>"; echo "</table>"; echo "</div>"; } elseif ($student == "false") { echo "<div align='center'>"; echo "<table width='325' height='175' border='0' cellspacing='0' cellpadding='0'>"; echo "<tr>"; echo "<td width='325' align='center' valign='middle'>"; echo "<p><font size='+1'><strong>Welcome!</strong></font></p>"; echo "<p><font color='#FF0000'>Incorrect Passcode! I have a table post for Date of Birth (Format YYYY-MM-DD) and a search form where you select From and To Numbers to find users between these ages. I have tried a few different things - this one works best, if I choose from any number and up til current time I get results but as soon as I set the script to use the selected variable I bomb. Also, somehow I got the script to only return the users with a DOB of 0000-00-00. Here is the current version (shortened to show the relevant parts): Code: [Select] $now = time(); //current timestamp $now=(date("Y-m-d")); if (isset($_POST['ageFr'])) //sets the stopdate for the search $ageFr = sanitizeString($_POST['ageFr']); $yearStop="date_sub($now, INTERVAL $ageFr YEAR)"; if (isset($_POST['ageTo'])) //sets startdate for the same search $ageTo = sanitizeString($_POST['ageTo']); $yearStart="date_sub($now, INTERVAL $ageTo YEAR)"; $query = "SELECT * FROM user_criteria WHERE sex='$sex' AND (age BETWEEN ($yearStart) AND ($yearStop))"; //The query When I echo like this: echo "$ageFr to $ageTo <br />"; echo "$yearStart - $yearStop"; echo "<br /> $query"; I get this: 18 to 101 date_sub(2012-01-24, INTERVAL 101 YEAR) - date_sub(2012-01-24, INTERVAL 18 YEAR) SELECT * FROM user_criteria WHERE sex='Female' AND (age BETWEEN (date_sub(2012-01-24, INTERVAL 101 YEAR)) AND (date_sub(2012-01-24, INTERVAL 18 YEAR))) Shouldn't that work? Am I making a big or a small mistake here? Pikachu? //calculate age $birthdate = "1978-04-26"; //birth date... actually being obtained from a database $today = date("Y-m-d H:i:s"); // The exact date $age = date_diff($str_birthday, $today); echo $age; I'd like a simple code to echo the age of someone with the mysql database information that's in their record. This doesn't work. I have no idea why. Nothing seems to work that I've found on the net. Please help. Thanks. I have an advanced search from that has 20 fields and it works great however I'm having a problem with customising the age range search based on the $birthdate field (YYYY/MM/DD). I want the form data to be 1) stored into a mysql database AND after storing said data, 2) email the same data to our sales department. 1) The form data DOES get stored into mysql database (except for the first two fields, for some weird reason) 2) I added a "mail" section to the php file that stores the data into the database, but it is not working correctly. I have stripped the email portion down to sending just one of the fields in the "message" to make it easier for troubleshooting I have included here, both the form section of the html file, and the formdata.php file that processes the data for your analysis. I am relatively new to php so there are going to be some issues with security, but I can work on those after I get the store & email process to work correctly. Please review my code and see if anyone can be of assistance. I looked through the forums and couldn't find another issue that was the same as mine. Hello everyone and thank you for taking the time to read this. I am very new to coding and I am helping a friend with a website. I'm trying to create a form with a <select> dropdown and filling the options with items from a database, there are very man items so I'm not sure 100% how to go about this, I have some code but I know it's wrong I'm just looking for a nudge in the right direction, any help is appreciated the code can be found below: Code: [Select] <div style="float:left;"> <select> $team = $sql->query("SELECT * FROM teams WHERE arenaid ='$platid'"); <option value="<?echo $teams>"></option> </select> </div> Again, thank you for taking the time to read this and thank you also for any help you can provide. Hi, I'm currently learning PHP through tutorials and I am working on a webpage that filters statistics (crime records) from an SQL table and filters them by an option of years that the user has picked from a dropdown box. $desiredyear = $_POST['year'] is the year picked from the drop down box on a different page. I did not encounter any problems simply displaying the entire table until I tried to filter the crimes by year. I keep getting this error message. Warning: sqlsrv_fetch_array() expects parameter 1 to be resource, boolean given in C:\RDEUsers\NET\400792\1.php on line 40 Here is the error line. Code: [Select] while($row = sqlsrv_fetch_array($results, SQLSRV_FETCH_ASSOC)) And here is my code so far Code: [Select] <?php $server = 'SQL2008'; $connectionInfo = array( "Database"=>"rde_400792"); $conn = sqlsrv_connect($server,$connectionInfo); $desiredyear = $_POST['year']; $describeQuery='select year, force, homicide from crime where year = $desiredyear '; $results = sqlsrv_query($conn, $describeQuery); echo '<table border="1" BORDERCOLOR=Black>'; echo '<tr><th bgcolor = "LightBlue">Year</th><th bgcolor = "LightBlue" >Force</th><th bgcolor = "LightBlue">Homicide</th></tr>'; while($row = sqlsrv_fetch_array($results, SQLSRV_FETCH_ASSOC)) { echo '<tr>'; echo '<td >' .$row['year'].'</td>'; echo '<td>' .$row['force'].'</td>'; echo '<td>' .$row['homicide'].'</td>'; echo '</tr>'; } echo '</table>'; sqlsrv_close($conn); Please can somebody help me fix this? It's driving me mad and I've exhausted my limited knowledge on the subject. I just don't understand what I'm doing wrong. |
