PHP - Select Which Fields To Display In Resultant Table
I have the following html code and php code. I would like clients to select which fields in the mysql database they would like to have appear in the resultant table by checking the text box next to the field in the html form. For instance they should be able to select first name and city and have only those columns appear in the resultant table.

Thank you in advance... here is my situation, I have a form with three (3) fields in it, the 'student name' is unlimited textfield with an "add more" button to it and I have two select fields ('number of shirts' and 'trophies') that depend on the number of entries for 'student name'...
I want to create the select fields based on this math, for as many 'student name' entries:
1- i want to have the select form for 'number of shirts' to be 0 up to that number... so if there are 6 'student name' entries, the select options will be 0,1,3,4,5,6,7
2- I want to have the select form for 'trophies' to be 5 'student name' entries to 1 'trophies', for example if there are 6 'student name' entries, the select options will be 0,1... if there are 13 entries, options will be 0,1,2... So if there are less than 5 'student name' entries, the select field will not show (hidden)
of course if there are no 'student name' entries, these two fields won't show up (hidden)
let me know if that make sense and ANY help or directions will be GREATLY APPRECIATED.

I'm looking for a way to have 5 different select menus. Each one pre-php-populated with the same data to begin with. Lets say numbers 1 through 5.
If user selects #2 in any of the lists, all the other lists #2 will dissapear from. With ultimately only one unique selection in each list (1 through 5).
Like having a list of 5 things to do and a select list next to each one to select the priority in which it gets done.
Is there a way to do this with ajax?

I'm using a small script (that I found online) to dynamically add fields to an order form.
It works well with normal text fields but I would like to use it with a select box fields (as it draws data from MySQL).
My problem is, that when the field is 'cloned' it contains the product id and not the product name - which is what the user needs to see.

I have a small table with 4 fields namely Day_ID, Dues, Last_Visit, Points. where Day_ID is an auto-increment field. The table would be as follows:
Day_ID -- Dues --- Last_Visit --- Points.

So this is the record of a person's visit to say a club. The last row indicates the last date of his visit to the club. His points on this date are 6. Based on this point value of 6 in the last row I want to retrieve all the previous BUT adjoining all records that have the same Points i.e. 6.
So my query should retrieve for me, based on the column value of Points of the last row (i.e. Day_ID - 6 ), as follows:

5 --------- 600 -------- 10/12 ------- 6
6 --------- 600 -------- 14/12 ------- 6
This problem stated above had been completely resolved, thanks to a lot of help from Guru Barand by this following query :-

As of now I can still get the results by running the query multiple times for each of the cv.points columns ( seperately for cv.points1, cv.points2, cv.points3) and it works correctly. However I am wondering if there is a better way to do this in just one go. This not only makes the code repetitive but also since the queries are interconnected, involves the use of transactions which I wish to avoid if possible. The values that I require for each of the cv.point columns is 1. day_id of the previous / old day on which the cv.points value changed from the current day value, and 2. cv.points on that old/ previous day. The website is www.prontoitaliano.co.uk/shop What needs to appear on the receipt are all the extras options from all the products(See checkboxes in all pasta dishes and checkboxes toppings in "create your own pizza") and 2 fields from the checkout page (Pick up Date, Pick up Time). To send the order to the printer I'm using the "Star cloud PRNT woocommerce" plugin and for the pickup date and time fields "ultimate woocommerce delivery and pickup date time", I've attached below the code that needs to be edited to add more fields to the receipt. Can someone please tell me how to use the "SHOW FIELDS FROM $mytable" in php? I understand how the MySql works but need to get these fields back into an array in php. I am currently creating a form and I want to populate a drop down selection menu with data from two fields in a form. For example, I want it to pull the first and last name fields from a database to populate names in a drop down menu in a form. I want the form to submit to the email address of the person selected in the drop down. Is this possible to do? The email is already a field in the record of the person in the database. Can anyone give me some pointers or advice on how I should go about setting up the "Select" box drop down? I am not sure how to code it to do what I am wanting.
