PHP - Header Function Redirects To Wrong Path

So I'm working for someone, and they want me to fix this error in a PHP file..

Can anyone tell me the error here, can how to fix it?

For the includes I use the following:

 include($_SERVER['DOCUMENT_ROOT'] . "/cms/includes/connection.php");

And for these following examples I use "http://localhost/":

<img class="header_image" src="http://localhost/cms/assets/image/capture.png" />
<script type="text/javascript" src="http://localhost/cms/jquery/jquery.js"></script>
<link rel="stylesheet" type="text/css" href="http://localhost/cms/assets/style.css" />

With these examples I use "http://localhost/" because "$_SERVER['DOCUMENT_ROOT']" is not working with these above.   My Question: Is there a function I could be using and replace it with "http://localhost/"?   I am looking to have it like following: When I upload the web app onto the web server it still should be working out.     EDIT:   "Un-linking" those links which happen did not work.

When ever I put it on my php page I get a blank page. I haven't been able to call the function yet.

Have I made this function correctly?

$info = fetch_user_info_by_id(); is an array.  Can I just pull check the values with if (($info['totalrequested'] === 0) && ($info['accounttype'] === 1)) or do I need to do something else?

I need to know if what I am doing will even work.

I have a .php page which submits information into a form to a insert_form.php page which then parses then information and adds it to a DB. In this insert_form.php page I have a function which is being called which is located in another .php file called function.php. This function adds images to the file system and will display errors if anything happens to the user. I do this by doing a header redirect:

The problem is the function is working because the images are being loaded correctly, but if something happens to the images and the header redirect gets initiated, its not redirecting.

I need to know if the header redirect would work when being a function in another file or not.

First, I would like to say when i tried to recover my account from this website, I took me 10 attempts to get the captcha right and then finally it said it sent me an email to my gmail account. I checked spam folder and everything there was no such email from this site. Then I decided to create a new account, well, it took me another 10 attempts to get the captcha right and finally when it was submitted, the page was loading for around 3 minutes before it signed me in.   My question is about the php date() function. It accepts a format to display a time. In the following example I use F for full representation of month, d for 2-digit day of month with leading zeros, Y for full year, g for 12-hour format without leading 0s, s for seconds and A for meridiem. It uses the correct format, but it gives me the wrong time. My local time is 4:43 and it prints out 4:12:   <?php

?>   Why is it 30 minutes behind?

I think it is unusual but i dont know to fix it.  when my computer clock shows 16: 13 hours , date() reuturns 10:43:16 . My systems time zone is +5.30 GMT.
how does date() subtracts 5.30 hrs from the system time?

Hi i turned my output_buffering to true on my php.ini because i need some of its features for trailing the system my problem now is when im getting .csv file using header function, it seems that it gives me the source code of the interpreted code instead of the expected .csv contents.

Im getting a warning with in_array.I have used if() inside for loop to check if value has already been printed or not. If not it will print and save the value in $done[].Can someone please point me out my mistake that i am getting a warning.

On my local computer running wamp my script works perfect and the pdf is generated without problems
