PHP - Topic Name: How To Store Form Array Data To Mysql?

I have a MySQL database table that has multiple records which look like (1, Which of these are your favorite color(s)?, Red||Blue||Orange||Green, question-type1 ) I have written some PHP code in extracting that data into a HTML form. (The above data looks like a question with multiple options (radio buttons/checkboxes) below it). Below is what I'm trying to achieve: When the action is Submit: store the question number, user responses of the options , time stamp and user name into a new database table. I came to know that passing arrays will do the job, but I got stuck in the middle. Please see the attached documents that has the code. I have stripped the email portion down to sending just one of the fields in the "message" to make it easier for troubleshooting I have included here, both the form section of the html file, and the formdata.php file that processes the data for your analysis. I am relatively new to php so there are going to be some issues with security, but I can work on those after I get the store & email process to work correctly. Hey, sorry for these fundamental array questions lately, I am not going to ask much more when it comes to this.
The hashtags come from the submit form (separated by commas) and get stored inside an array with the "explode()" function. The script should select the ID numbers of the hashtags from the table.
With the script above only the last row gets inserted into $fetch_array inside the while loop.
When the $fetch_array variable is printed on screen then only one ID number can be seen inside the array:

I'm working on a store locator style program. I first get and sort all zip codes based on a their distance to an original location. I then use that zip code array to find all stores in the mysql database, but when I get the results, they're no longer arranged by distance from the original location.. and I can't just sort them ascending or descending based on their zip codes because that doesn't sort them correctly either. The topic_order field is a new field I added to get the desired output but it's not standard procedure and is in fact not really workable in a live setting because I would have to use php_myadmin to modify it everytime I added or deleted a topic or sub-topic. So the bottom line is that I can't figure out anyway to code the script to always show the sub-topic right under the associated main topic and all in order. If I add a sub-topic to one of the upper main topics it shows up at the bottom; hence the addition of the topic_order field. So right now it's basically a mess and I can't figure out how to code everything to work correctly. I have searched the forums here as well as several other sites and cannot get any clues.
I've got a form on my site, with a textbox...and the info is sent to a db. How would I get the textbox info to be displayed the same as it was entered? I set a one <form> on every form field I have including the submit button. Now whenever I press the submit button it redirects to insertvalues.php which it should be doing. In insertvalues i told it to query the form data and post it into my database's table. Its not doing that and tells me that it has a database error which i set it to tell me if something goes wrong. I have the site set up like product.php?Item=26 and the item number changes based on the product. The page items are being populated with info stored in the mysql databse.
I need the email being sent to say something like
