PHP - Dynamically Add Form Fields (from A Select Box)

I'm using a small script (that I found online) to dynamically add fields to an order form.
It works well with normal text fields but I would like to use it with a select box fields (as it draws data from MySQL).
My problem is, that when the field is 'cloned' it contains the product id and not the product name - which is what the user needs to see.

Thank you in advance... here is my situation, I have a form with three (3) fields in it, the 'student name' is unlimited textfield with an "add more" button to it and I have two select fields ('number of shirts' and 'trophies') that depend on the number of entries for 'student name'...
I want to create the select fields based on this math, for as many 'student name' entries:
1- i want to have the select form for 'number of shirts' to be 0 up to that number... so if there are 6 'student name' entries, the select options will be 0,1,3,4,5,6,7
2- I want to have the select form for 'trophies' to be 5 'student name' entries to 1 'trophies', for example if there are 6 'student name' entries, the select options will be 0,1... if there are 13 entries, options will be 0,1,2... So if there are less than 5 'student name' entries, the select field will not show (hidden)
of course if there are no 'student name' entries, these two fields won't show up (hidden)

I am currently creating a form and I want to populate a drop down selection menu with data from two fields in a form. For example, I want it to pull the first and last name fields from a database to populate names in a drop down menu in a form. I want the form to submit to the email address of the person selected in the drop down. Is this possible to do? The email is already a field in the record of the person in the database. Can anyone give me some pointers or advice on how I should go about setting up the "Select" box drop down? I am not sure how to code it to do what I am wanting. Any links to relevant help would be appreciated too. Thanks in advance! I'm trying to create a table form that pulls in "Extra" choices from a table and allows users to select a number of days and number of guests for each choice. I started creating it a a checkbox field but am thinking just using the name as a label and setting days and guests to default "0" might be a better bet. Guidance from someone more experienced with this? Also, how do I store the submission (db structure - in the "reservations" table, another?). I am creating a dropdown box on an edit user form. The box is populated from a table ssm_role and relates to the user which is another table. I am wanting the value of the dropdown to be set as whatever the loaded user has in their database table. I have a calendar select date function for my form that returns the date in the calendar format for USA: 02/16/2012. I need to have this appear as is for the form and in the db for the 'record_date' column, but I need to format this date in mysql DATE format (2012-02-16) and submit it at the same time with another column name 'new_date' in the database in a hidden input field. Is there a way to do this possibly with a temporary table or something? Any ideas would be welcome. Doug

If user selects #2 in any of the lists, all the other lists #2 will dissapear from. With ultimately only one unique selection in each list (1 through 5).
Like having a list of 5 things to do and a select list next to each one to select the priority in which it gets done.

The problem comes when I need to get the value of each field input field and pass it into a query. The 'value' field of the inputs are numbers, such as 68, 70, 124, 108 etc so I can't get them in a for loop etc What's the easiest way to get all of these input field values? Can I use $_POST? I was thinking of adding queries using a basic HTML form. To be able to do that I was thinking of having a Query sql table which has all the main create, insert, remove, update, delete and select general syntax into it. The idea is to be able to choose a query based on a HTML select option and add the values to a field, any idea how can you do that dynamically?

I need to search in two tables (news and pages), I already had sucess doing my search system for just one table, but for two tables its not easy to do.
I already use a select statment with two tables using UNION because I want to show number of search results, that is number of returned rows of my first sql statment.
But now I need to do a select statment that allows me to acess all fields of my news table and all fields of my pages table.
I need to acess in my news table this fields: id, title, content, link, date, nViews
I need to acess in my pages table this fields: id, title, content, link
Im trying to do this also with UNION, but in this case Im not having any row returning.

However if you delete rows 3, 4 and 5 and submit the page shows a value of 0.00 in the total column on rows 3, 4 and 5. How do i get this value removed so it is blank same as the rest ?
