PHP - Adding 'request Category' Form
I'm working on a business listing website using a Wordpress theme as the framework.
Unfortunately, there is a limited number of categories with no option for a user to request a category. Hey guys. Right ill try explain as best i can. Basicly we were told to write a php script that can view a shopping cart. The items in the catalogue should be stored as an array of objects. (The next assignment will require you to store this list of objects in a database). There should also be pages to allow the user to add, edit and delete items in the catalogue. So I've been developing an app mostly in PHP, but am rather afraid of JS. Hope to fix that.
I have an AJAX dropdown using JQuery to search locations. It works great. However, I want to make it similar to what is seen on this site:

As you can see, the dropdown, when clicked populates a box. Then the user submits the form and the data is used in the application.
I have no clue how to make the form populate with data from the DB (I'm using mySQL) when clicked. So far, I've only been able to make it clickable as a URL (not what I want, obvioiusly!)
Is there a way to do this on a really small, simple script for starters? I'm certain their is, but don't even know where to begin.

I'm trying to add an upload feature to my contact form. I had one before that simply added the image under the text in the email not as an actual attachment and it was pretty easy but I lost the code and can’t seem to find it again. But I seem to be having a problem finding a form that can help me add this. As of now the code in HTML has the upload but the PHP doesn’t at all.
If anyone could help me add the rest of the PHP code I'd really appreciate it!

Trying to fix existing some code (complete beginner here) on the company I work for website. There's a check-box which when ticked should apply delivery charges and if not it shouldn't - currently it's working in that on the same page it calculates the total cost correctly when checked or not checked but when passing on information it always passes on the delivery charge even if un-ticked... The simplest thing I can see would be to put an if statement in to say if (not ticked) then del_charges = 0.00. Any idea how I can do this in this instance?
