I have an AJAX dropdown using JQuery to search locations. It works great. However, I want to make it similar to what is seen on this site:

As you can see, the dropdown, when clicked populates a box. Then the user submits the form and the data is used in the application.
I have no clue how to make the form populate with data from the DB (I'm using mySQL) when clicked. So far, I've only been able to make it clickable as a URL (not what I want, obvioiusly!)
Is there a way to do this on a really small, simple script for starters? I'm certain their is, but don't even know where to begin.

I have a html form that is used to submit property details, There is multiple form inputs and also I need to upload multiple property images using Dropzone JS multiple image upload. Here I'm validating form inputs using jQuery Validation library. Validation works perfect and data Is being to posted to php file called submit_property_data.php. But when I implement the Dropzone JS image upload its not working.

The idea behind this is jquery/ajax is going to get passed an id (eventid) from a dropdown select box inside a form and it's going to run a query to find all the events in my database with that same id. After running the query it's going to list them out and find the highest number of another field (label) and its going to add one to its value and then what I would like for it to is send it back to the form in the jquery/ajax and and place that label inside of the existing input text field in the form. I have a form that is used to create listings on a website and it has text fields and a dropzone.js file uploader in it. But on submit of the completed form, i would like it to upload the files and submit the text fields to mysql. The upload files part is working and also the mysql part working but I cannot work out how to make the relationship between the listing and the images that are attributed to the listing, which are stored in different tables. At the point of submission the user is not logged in, so i cannot use that to assist with telling mysql where the image is attributed to.. I have looked into submitting the text form first then redirecting to the upload section but it really isn't what I want for the form. It would be nice that the user can submit all the information for that listing in one form. Now i am sure that there is a really simple way to achieve this but i am running round and round in circles on this one.

Any help would be a much appreciated Which one is better for standards practices in PHP. 1. Using the same form for everything. (Add and edit). Meaning setting up one form to handle adding new records, as well as editing existing records. Or 2. Using two different forms for both actions. Use one form/area to handle Adding, and one form/area to handle editing. Which one of these are better from a standards/practice point of a view. Which one better fits into the MVC platform (a framework like Codeignitor, or Cake). I am creating a user inbox system. I am retrieving all the unread messages. Each message row contains a "reply" form. So say I have 10 messages showing on a single page. That's 10 forms. What I would like to know is how can I submit any one of the 10 forms and not have it affect the remaining 9 forms? I have read around and can't seem to find the right coding for what I need on this forum and some other other forums. I have a contact form (as listed below) and I need 2 locations (Print Name and Title) fields to auto-populate on a separate form (can be a doc, pdf, etc. any form of document which is easiest) and this form can be totally back end and the individual using the form never is going to see the form. It's going on a contract form, that we would like to auto-populate.
